Our Major Medical plans (no limits) use 3 networks: PHCS, Cigna, and Anthem.
Network selection affects pricing as does the deductible selected.
Deductible (In-Network) – Ind/Fam: start at $1,000/$2,000 to $7250/$14,500
Deductible (Out-of-Network) – Ind/Fam: $2,000/$4,000 to $14,500/$29,000
Out-of-Pocket Limit (In-Network) – Ind/Fam: $9,200 / $18,400
Out-of-Pocket Limit (Out-of-Network) – Ind/Fam: $18,400 / $36,800

Activating your Healthcare:
- Visit https://lifex.ushealthcenter.com/app
- Select Activate Your Account.
- Select Activate by Demographics.
- Enter your information and click Activate.
- You will receive your username. (Remember this for future logins!)
- Select Login and enter your password.
- Congratulations! Your PHD account is now activated.
PHCS Life X Doctor Lookup
- Go to https://www.multiplan.com/webcenter/portal/ProviderSearch
- Click “Select Network” and choose “PHCS”. Then select “Practitioner & Ancillary”
- Type your zip code into the “City/County & State or Zip” box
- Select what type of doctors you want to look up in the box to the left of the zip code. Example: Select family practice
- Hit the search button and a list of doctors will populate the PHCS/MultiPlan PPO in your area.
Anthem Life X Doctor Lookup
- Visit https://www.anthem.com/find-care/
- Click “Basic search as a guest”
- In the first dropdown menu, select Medical Plan or Network (may also include dental, vision, or pharmacy benefits).
- In the second dropdown menu, select the state where the plan or network is offered. (For employer-sponsored plans, select the state where your employer’s plan is contracted in. Most of the time, it’s where the headquarters is located.)
- In the third dropdown menu, select Medical (Employer-Sponsored).
- In the last dropdown menu, select National PPO (BlueCard PPO). Click Continue.
- Enter your location and search criteria or search by provider.
